INSTRUCTIONS: CONNECTICUT BUSINESS LICENSE PERSONNEL LIST (Form K-26)

As part of the documentation required for a Connecticut business license issued by the dealers and repairers division of the state department of motor vehicles, a form K-26 should be completed. This document can be obtained from the website of the government of the state of Connecticut.

Connecticut Business License Personnel List K-26 Step 10: Enter your Department of Environmental Protection waste disposal number, if used, in the tenth blank box.

Connecticut Business License Personnel List K-26 Step 12: Enter any franchises in the twelfth blank box, which is only to be completed by new car dealers.

Connecticut Business License Personnel List K-26 Step 13: Indicate whether the company is owned by an individual, a partnership, a corporation or a LLC with a check mark.

Connecticut Business License Personnel List K-26 Step 14: The next section requires you to document owners, partners, members, managers, managing members or corporate officers. For each, provide their name, position with the business, home address, date of birth, Social Security number and home phone number.
