To apply for registration for payment of Florida business tax, a form DR-1 should be filed. This document can be obtained from the website of the Florida Department of Revenue.

 

Florida Business Tax Application (Formerly, Application To Collect And/Or Report Tax In Florida DR-1 Step 1: The first page contains Section A, which concerns your reason for applying and requires you to provide information about your business entity.

 

Florida Business Tax Application (Formerly, Application To Collect And/Or Report Tax In Florida DR-1 Step 2: The second page requires you to provide further information about your business entity, including your structure and ownership and background information.
