On this document, state sales tax and tax due to the cities of Atlanta, Clayton and Muscogee are computed. Lines 11 through 17 provide additional space for documentation. An ST-3 addendum is used to document business in additional jurisdictions. This document can be obtained from the website maintained by the Georgia Department of Revenue. Additional instructions are available in the form ST-3 packet also available on their website.
