Sec. 10a-150c. Content of disclosure statement. Such disclosure shall be made
not later than January thirty-first of each year and shall include the following information
for the preceding year:
(1) The amount or value of the gift and the date when it was made;
(2) Full details of the conditions, matching provisions or designation of the gift if
it was made on a conditional or matching basis or designated for a particular purpose;
(3) In the case of a gift by a foreign government, the name of such government and
in the case of a gift by a foreign person or entity, the name of the foreign country which
is the principal residence or location of such person or entity; and
(4) The purpose for which the gift will be used.
