40-2131. Same; health service provider agreements; charge to insured notpermitted, when.Unless otherwise specified by the plan, as a prerequisite forpayment from the plan, each provider of health services to persons coveredunder the plan shall enter into a provider agreement with the association underwhich reimbursement for services provided shall be at rates established by theboard.Providers shall not charge persons covered under the plan with the exceptionof authorized deductible and co-pay requirements and noncovered services if therecipient has been informed in advance of the noncoverage.
