      58-4108.   Application for certification, licensure or examination.(a) Applications for original certification and licensure and renewal ofcertificates and licenses shall be made in writing to the board on formsapproved by the board and shall be accompanied by the appropriate feesprescribedpursuant to K.S.A. 58-4107 and amendments thereto.

      (b)   Applications for examination shall be made in writing to the board onforms approved by the board and shall beaccompanied by theappropriate fee prescribed pursuant to K.S.A. 58-4107 andamendments thereto. If a testingservice has been designated by the board to administer the examination,applications for examination shall be made in writing to the testingservice on forms approved by the testing service.

      (c)   At the time of filing an application for certification or licensure,eachapplicant shall sign a statement agreeing to comply with the standards setforth inthis act and stating that the applicant understands the types of misconductfor which disciplinary proceedings may be initiated against a statecertified or licensed appraiser, as set forth in this act.
