Same; how reported.In reporting every birth and stillbirth, physicians and others permittedto attend pregnancy cases and required to report births and stillbirthsshall state on a separate sheet accompanying the birth certificate orstillbirth certificate, as the case may be, whether a blood test forsyphilis has been made during such pregnancy upon a specimen of blood takenfrom the woman who bore the child for which a birth or stillbirthcertificate is filed, and if made the date when such test was made, and ifnot made, the reason why such test was not made. Neither the fact that sucha test was or was not made or was or was not required by law nor the resultof any such test shall appear upon the birth certificate or be certified toany person for any purpose.
