      65-4966.   Patient entitled to receive copy of contactlens prescription; disclosures; prescription limitations.Each patient shall be entitled to receive a copy of suchpatient's contact lens prescription once the same prescription has beendetermined and the adaptation period has been completed. Any prescription for aspecific brand of contact lenses available only from the licensed optometristor personlicensed to practice medicine and surgery, but which are generally marketedunder an alternate brand, must disclose the name of the manufacturer and thetrade name of the alternate brand. No contact lens prescription may be limitedby an expiration date or otherwise to a period of less than 12 months fromeither the date the prescription is first determined or the last date of thecontact lens evaluation by a licensed optometrist or a person licensed topractice medicineand surgery, whichever date is later, unless a health related reason for thelimitation is noted in the patient's medical record.
