Article 5.--MATERNITY CENTERS AND CHILD CARE FACILITIES

      65-507.   Records of maternity centers and child carefacilities; confidentiality.(a) Each maternity centerlicenseeshall keep a record upon formsprescribed and provided by the secretary of health and environment and thesecretary of social and rehabilitation services which shall include the name ofevery patient, together with the patient'splace of residence duringthe year preceding admission to the center and thename and addressof the attending physician. Eachchild care facility licensee shall keep a recordupon forms prescribed and provided by the secretary of health andenvironment which shall include the name and age of eachchild received and caredfor in the facility; the name of the physician who attendedany sick children in the facility, together with the names and addresses of theparents or guardiansof such children; and such other information as the secretary of healthand environment or secretary of social and rehabilitation services may require. Eachmaternity center licensee and each child care facility licenseeshallapply to and shall receive without charge from thesecretary of health andenvironment and the secretary of social and rehabilitation services formsfor such records as may be required, which forms shall contain a copy of thisact.

      (b)   Information obtained under this section shall be confidential andshall not be made public in a manner which would identify individuals.
