27A.430 Institutional level. The institutional level of the system shall consist of at least the following information: <br>(1) Date of institutionalization; <br>(2) Type of incoming action; <br>(3) If defendant sentenced to death: (a) First scheduled date of execution; <br>(b) Date defendant executed; <br>(c) Date sentence commuted in lieu of execution; <br>(d) Sentence to which sentence of death was commuted; (4) Date defendant released from institution; <br>(5) Type of release from institution; <br>(6) If the offender is released on parole: (a) Offense for which convicted; <br>(b) Maximum expiration date; <br>(c) Minimum expiration date; <br>(d) Was the parole supervision fee imposed; <br>(e) What was the amount actually imposed for the parole supervision fee; <br>(f) What amount of the parole supervision fee was actually collected; <br>(g) Was restitution ordered as part of conditions of the parole; <br>(h) What amount of restitution was ordered; <br>(i) What amount of restitution has been paid; <br>(j) Was a victim impact statement presented to the parole board; did it favor the release of the offender; (k) Did the prosecutor present a statement to the parole board; did it favor the release of the offender; and (l) Did the victim or a representative of the victim appear before the parole board; did he favor the release of the offender; (7) If the offender released on parole violates parole or is rearrested: (a) What was the specific parole violation; <br>(b) Was the offender arrested for the violation; <br>(c) What was the outcome of the parole violation hearing; <br>(d) Was the offender reinstitutionalized; <br>(e) If arrested for a new criminal offense, list the KRS number, name, and level of the offense; (f) Was the offender subsequently convicted thereof; <br>(g) Was the offender reinstitutionalized for the offense; and <br>(h) Was the offender placed on probation for the offense.
