§ 5-575. Applications for licenses.
(a) In general.- An applicant for a license shall submit an application to the Department on the form that the State Superintendent requires.
(b) Contents.- An application for a license shall contain:
(1) the name of the applicant;
(2) the proposed location of the child care center;
(3) the name of the individual to be in charge of the child care center; and
(4) any other information that the Department requires.
