333.5823 Eligibility for services; application; financial investigation; medical evidence; copy of reports.
Sec. 5823.
When a crippled child is found whose condition can be treated and whose parent or spouse is unable to provide proper care and treatment, a person authorized by rule shall apply to a designated representative of the department for eligibility for services under this part. The representative shall make a financial investigation and secure medical evidence as to the condition of the child. A copy of the report of the financial investigation and the report of a physician or dentist, if any, shall be sent to the department.
