Application for special use permit.
229.310. The application for such special use permit shallbe made by the party in interest at an office of the countyhighway engineer and surveyor on blanks kept by him and furnishedby the county. The applicant shall state his name, residence,and business address, the purpose for which the application ismade, giving the exact location, or locations, of the street,avenue, boulevard, road, alley, public easement, or highwayinvolved, the nature of the proposed use, the starting time andthe approximate time desired to complete the particular use andthat he will restore and replace such street, avenue, boulevard,road, alley, public easement, or highway, together with any otherand further information which may be deemed necessary andpertinent by the county highway engineer.
