330.050. 1. Upon due application therefor and upon submission bysuch person of evidence satisfactory to the board that such person islicensed to practice podiatric medicine in this state, and upon the paymentof the fees required to be paid by this chapter, the board shall cause tobe issued to such applicant a license, which license shall recite that theperson therein named is duly registered for the period specified.

2. Such license shall contain the name of the person to whom it isissued and the office address of such person, the date and number of thelicense issued to such person to practice podiatric medicine.

3. Every person practicing podiatric medicine shall, upon receivingsuch license, cause the same to be conspicuously displayed at all times inevery office maintained by such person or in which he practices podiatricmedicine in this state. If such person maintains or practices in more thanone office in this state, the board shall issue to such person a duplicatelicense for each office so maintained or in which such person may practice.If such person maintains or practices in more than one office in thisstate, the board shall issue to such person duplicate renewal licenses foreach office so maintained or in which such person may practice.

4. If any registrant shall change the location of his office duringthe period for which any license shall have been issued, such registrantshall, within fifteen days thereafter, notify the board of such change,whereupon he shall be issued, without additional fee, a duplicate renewallicense for such new location.
