351.047. The secretary of state may prescribe and furnish on requestforms for all documents required or permitted to be filed by this chapter.The use of the following forms is mandatory:
(1) A foreign corporation's application for a certificate ofauthority to do business in this state;
(2) A foreign corporation's application for a certificate ofwithdrawal;
(3) A corporation's corporate registration report.
