Duplicate forms, when furnished.
351.155. It shall be the duty of the secretary of state to furnishforms of corporate registration reports to any corporation upon request toany representative of the corporation, but no such form of the corporateregistration report shall be furnished unless the name of the corporationfor which it is desired shall accompany the request.
