354.720. 1. Every prepaid dental plan corporation shallfile with the director annually, on or before March first, areport verified by at least two principal officers covering thepreceding calendar year.
2. Such report shall be on forms prescribed by the directorand shall include:
(1) A financial statement of the corporation, including itsbalance sheet and receipts and disbursements for the precedingyear;
(2) Any material changes in the information submittedpursuant to section 354.704;
(3) The number of persons enrolled during the year, thenumber of enrollees as of the end of the year, and the number ofenrollments terminated during the year.
