Prescription drugs, one co-payment for dosage prescribed.

376.386. For any health carrier or health benefit plan, as defined insection 376.1350, that provides prescription drug coverage, if aprescription drug covered by a health carrier or health benefit plan isprescribed in a single dosage amount for which the particular prescriptiondrug is not manufactured in such single dosage amount and requiresdispensing the particular prescription drug in a combination of differentmanufactured dosage amounts, the health carrier or health benefit planshall only impose one co-payment for the dispensing of the combination ofmanufactured dosages that equal the prescribed dosage for such prescriptiondrug. Such co-payment requirement shall not apply to prescriptions inexcess of a one-month supply. If technology does not permit suchadjudication, the health carrier or health benefit plan shall providereimbursement forms for the patient.
