Requests by department for information, form--content.
660.335. Requests made by the department pursuant to sections 660.327and 660.330 shall be in writing and shall include sufficient information toidentify each person named in the request in a form that is compatible withthe record-keeping methods of the entity. Requests may be made in the form ofmagnetic computer tape.
