1609 - Application for commercial driver's license.

     § 1609.  Application for commercial driver's license.        (a)  Contents of application.--The application for a     commercial driver's license or commercial driver learner's     permit shall include the following:            (1)  The full name and current residential address of the        person.            (2)  A physical description of the person, including sex,        height and eye color.            (3)  Date of birth.            (4)  The applicant's Social Security number.            (5)  The person's signature.            (6)  Certifications, including those required by Federal        regulations.            (7)  Any other information required by the department.        (b)  Change of name or address.--Whenever any person, after     applying for or receiving a commercial driver's license or     commercial driver learner's permit, moves from the address named     in the application or in the driver's license or learner's     permit issued or when the name of the licensee or permittee is     changed, such person shall, within 15 days, make application for     a duplicate license. The duplicate shall be issued upon payment     of the required fee and return of the original, or previous     duplicate, license.        (c)  New residents.--No person who is a resident of this     Commonwealth for 30 days shall drive a commercial motor vehicle     under the authority of a commercial driver's license issued by     another jurisdiction.        (d)  Waiver.--Notwithstanding the provisions of subsection     (a)(4), the department shall not require a Social Security     number on the application for a commercial driver's license of a     person who submits a waiver obtained from the Federal Highway     Administration of the United States Department of Transportation     permitting him not to have a Social Security number. If the     waiver requires the person to have some other sort of identifier     in place of the Social Security number, the identifier must be     indicated on the application.
