§ 23-4.11-5 - Recording determination of terminal condition and contents of declaration.

SECTION 23-4.11-5

   § 23-4.11-5  Recording determination ofterminal condition and contents of declaration. – Upon determining that the declarant is in a terminal condition, the attendingphysician who has notice of a declaration shall record the determination andthe substance of the declaration in the declarant's medical record.
