SECTION 5-70-15

   § 5-70-15  Certificate –Identification cards. – (a) Upon payment of the fees required by this chapter, an individual who hascomplied with the requirements of this chapter is entitled to a certificate andidentification card indicating that he or she is qualified for licensure underthe provisions of this chapter.

   (b) Each certificate shall contain the name of the individualto whom it was issued and his or her date of birth and the category orcategories to which he or she is licensed.

   (c) Each individual identification card shall contain thename of the individual to whom it was issued, his or her date of birth, theclass of license, the category or categories to which he or she is licensed,and a head and shoulders picture (passport type) taken within three (3) monthsprior to the date of issuance of the license.

   (d) A duplicate certificate and/or identification card toreplace one which has been lost, destroyed, or mutilated may be issued by theboard upon payment of the fee required by this chapter.
